Patients enrolled at the Lakeshore Community NPLC have access to comprehensive primary health care including annual physicals, episodic illness care, immunizations, smoking cessation, injury prevention and monitoring and management of chronic diseases such as diabetes, arthritis, asthma, heart disease and mental health conditions.
Nurse Practitioner-Led Clinics are interprofessional teams. In addition to nurse practitioners, our team includes registered nurses, social worker, dietician, physiotherapist and health promoter. Through a partnership with the Best Care Lung Program patients have access to a respiratory therapist. The Windsor Essex Community Health Centre offers diabetic counselling to patients at our location.
