We are dedicated to listening, learning, and responding to your comments, suggestions, or compliments as we strive to deliver the highest quality health care in a welcoming and supportive environment. Your satisfaction with your care is very important to us.

If you are an existing patient and would like to share a concern, suggestion, or compliment, you have several options:

Speak with Your Provider: We encourage you to first discuss your concerns directly with your provider. You can book an appointment through reception for this purpose. A strong provider–patient relationship is built on openness, transparency, and trust.

Submit Written Feedback: Print and complete this form, and return by mail, fax or drop-off at the office.
